Main Classes of Diabetes Medications

Metformin is the most commonly prescribed first-line diabetes drug. It does not cause weight gain and carries a low hypoglycemia risk. Sulfonylureas (glimepiride, glyburide) stimulate insulin secretion and carry a significant hypoglycemia risk. SGLT-2 inhibitors (dapagliflozin, empagliflozin) excrete glucose via urine and offer cardiovascular and kidney protection. GLP-1 agonists (semaglutide, liraglutide) are injectable medications that also cause weight loss. Insulin: used in type 1 diabetes and advanced type 2 — always carries hypoglycemia risk.

Recognising and Treating Low Blood Sugar (Hypoglycemia)

Symptoms: cold sweats, trembling, intense hunger, rapid heartbeat, dizziness, confusion. Immediate action: take 15–20g of fast-acting glucose — glucose tablets, 150ml fruit juice, or 1 tablespoon of sugar. Recheck blood sugar after 15 minutes; repeat if still low. If the person is unconscious, never give anything by mouth — call emergency services immediately. Alcohol is especially dangerous because it both lowers blood sugar and masks hypoglycemia symptoms.

Metformin + Alcohol and Contrast Dye Warnings

Metformin + alcohol: Both increase lactic acid production. Combined, they can cause lactic acidosis — a rare but life-threatening condition. Avoid heavy drinking while on metformin. Metformin + CT contrast dye: In patients with reduced kidney function, contrast dye can cause acute kidney injury that allows metformin to accumulate to toxic levels. Always tell your doctor you take metformin before any CT scan with contrast — temporary discontinuation may be required.

Drug Interactions With Diabetes Medications

①Corticosteroids (prednisone): Significantly raise blood sugar, making diabetes control very difficult. ②Thiazide diuretics: Can raise blood sugar levels. ③Beta-blockers: Mask hypoglycemia warning signs (trembling, palpitations), making dangerous low blood sugar harder to detect. ④NSAIDs (ibuprofen): Can impair kidney function and reduce metformin clearance. ⑤Fluoroquinolone antibiotics (ciprofloxacin): Can cause unpredictable blood sugar fluctuations.

Diet and Lifestyle

Eating at regular times is critical — skipping meals increases hypoglycemia risk for those on insulin or sulfonylureas. Choose complex carbohydrates (whole grains, vegetables) over refined ones (white rice, white bread, sugar). Alcohol lowers blood sugar unpredictably — if you drink, always eat at the same time and check your glucose more frequently. Regular exercise improves insulin sensitivity and helps blood sugar control.
