Ibuprofen Safety Guide — Stomach, Kidney & Heart Risks Explained
Ibuprofen effectively relieves pain, fever, and inflammation, but carries risks of gastrointestinal bleeding, kidney injury, and cardiovascular events. Know the limits before you take it.
How Ibuprofen Works
Ibuprofen belongs to the non-steroidal anti-inflammatory drug (NSAID) class. It works by inhibiting cyclooxygenase (COX) enzymes, which blocks the production of prostaglandins — molecules responsible for pain, fever, and inflammation. Unlike acetaminophen, ibuprofen has genuine anti-inflammatory effects, making it particularly useful for arthritis, muscle injuries, menstrual cramps, and dental pain.
Recommended Dose
OTC adults: 200–400 mg per dose, maximum 1,200 mg per day. Prescription doses can reach 3,200 mg per day under medical supervision. Always take ibuprofen with food or milk to reduce stomach irritation. Maintain at least 4–6 hours between doses.
Gastrointestinal Risks
Ibuprofen suppresses prostaglandins that protect the stomach lining, increasing the risk of gastritis, ulcers, and GI bleeding. Never take on an empty stomach. If you experience severe heartburn, taking an antacid or proton pump inhibitor (e.g., omeprazole) alongside may help — but ask your pharmacist first. Seek immediate medical attention if you notice black tarry stools or persistent abdominal pain (signs of bleeding).
Kidney & Cardiovascular Risks
NSAIDs reduce blood flow to the kidneys, which can cause acute kidney injury especially when dehydrated, elderly, or if you already have kidney disease. Long-term or high-dose NSAID use can raise blood pressure and increase the risk of heart attack and stroke in patients with existing cardiovascular disease. If you have chronic kidney disease, heart failure, or hypertension, consult your doctor before using ibuprofen.
Drug Interactions to Avoid
①Aspirin (low-dose cardiac): Ibuprofen can block aspirin's antiplatelet effect. If you take daily aspirin for heart disease, take aspirin first and wait at least 30 minutes before ibuprofen. ②ACE inhibitors / ARBs (blood pressure meds): NSAIDs reduce their effectiveness and may cause kidney problems. ③Lithium and methotrexate: Ibuprofen raises blood levels of these drugs, increasing toxicity risk. ④Warfarin: Combination significantly raises bleeding risk.
